Acknowledgment of Risks:

I, the undersigned, acknowledge that participation in boxing and related activities at DB3 Boxing Gym involves inherent risks. These risks include, but are not limited to, bruises, fractures, sprains, cuts, concussions, and other severe injuries, including the risk of permanent disability or death. I fully understand and appreciate the nature of these risks.

Assumption of Risk:

I voluntarily agree to assume all risks associated with participation in boxing and any related activities at DB3 Boxing Gym. I understand that these risks may be caused by my actions or inactions, the actions or inactions of others, or the conditions of the premises or equipment used.

Medical Fitness:

I certify that I am physically fit and have no medical conditions that would prevent me from safely participating in boxing activities. I understand it is my responsibility to consult with a physician before participating if I have any concerns about my health or fitness.

Release of Liability:

In consideration of being allowed to participate in boxing and related activities at DB3 Boxing Gym, I hereby release, waive, discharge, and covenant not to sue DB3 Boxing Gym, its owners, employees, trainers, agents, and affiliates from any and all liability, claims, demands, or causes of action that may arise from my participation in these activities. This release extends to claims arising from the ordinary negligence of the released parties but does not include claims arising from gross negligence or willful misconduct.

Parental Consent (for Participants under 18):

I, the undersigned, am the parent or legal guardian of the participant named above. I have read and fully understand the terms of this waiver and release of liability, and I consent to my child’s participation in boxing and related activities at DB3 Boxing Gym. I agree to the terms outlined above on behalf of my child.

Governing Law:

This waiver and release of liability shall be governed by and construed in accordance with the laws of the Commonwealth of Virginia.
